If you would like to register with the practice please use this form.

Before coming to the practice in order to register you should first check that you meet the registration criteria which is as follows:

  • You must be resident within our catchment area
  • You must be intending to stay within the catchment area for a period of more than 6 months.
  • You must be entitled to free NHS treatment.

Any patient wishing to register will be asked to produce proof of address and if possible a medical card.

Once you have established that the above criteria has been met, the practice should be contacted in order for a ‘new patient interview’ appointment to be booked. A sample of urine should be brought to this appointment where a clinical receptionist will test it and also record height, weight, blood pressure, family and personal history. Should a doctors appointment be required, one may be made at this point.

Please take note that all registration requests are accepted at the doctors discretion and the practice has the right to refuse a registration request. Any refusal to a registration request will be given in writing.

Ordinarily Resident

Anybody in England can register with a GP practice and receive free medical care from that practice.

However, if you are not ‘ordinarily resident’ in the UK you may have to pay for NHS treatment outside of the GP practice. Being ordinarily resident broadly means living lawfully in the UK on a properly settled basis for the time being. In most cases, nationals of countries outside the European Economic Area must also have the status of ‘indefinite leave to remain’ in the UK.

Some services, such as diagnostic tests of suspected infectious diseases and any treatment of those diseases are free of charge to all people, while some groups who are not ordinarily resident here are exempt from all treatment charges.

You may be asked to provide proof of entitlement in order to receive free NHS treatment outside of the GP practice, otherwise you may be charged for your treatment.
